AGREEMENT AND WAIVER

• KNPS reserves to the right to organize players into groups as we feel necessary to achieve the best results.
• By signing this document, you agree to the KNPS refund policy stated on our website unless other stated in direct communication with KNPS.

I agree that I shall provide health insurance to cover any personal injury and property damage sustained by the student while participating in any activities of or while on the premises of Urban Edge LLC. I hereby release, discharge Urban Edge LLC, Kurt Nichols Power Skating and employed, from all damages, suit, causes of action, or liabilities for personal injuries and or property damages which may arise as a result of any such accident or loss. By Signing this agreement and waiver, it is also agreed that express permission is granted to Urban Edge LLC to use photographs and videos in which the student’s image and likeness appear in connection with his/her participation of Urban Edge LLC under Kurt Nichols Power Skating. Said express permission shall apply to the display, publication, distribution, use, print and reprint of such images and likeness, and the right to employ such images or likeness in advertising and promotions.

Covid-19 Matters. The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Urban Egde LLC/KNPS has put in place preventative measures to reduce the spread of COVID-19; however, UE LLC/KNPS cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending a UE LLC/KNPS program could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you and your child(ren) may be exposed to or infected by COVID-19 by participating at in an Urban Edge LLC/Kurt Nichols Power Skating program and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 while participating in or at any clinic/program may result from the actions, omissions, or negligence of yourself and others, including, but not limited to, Program Providers’ employees, volunteers, and program participants and their families.

You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to you or your child(ren) (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you or your child(ren) may experience or incur in connection with your child(ren)’s participation during Urban Edge LLC/KNPS Programs. (“Claims”). On my behalf, and on behalf of my children, you hereby release, covenant not to sue, discharge, and hold harmless the Program Providers, their employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. you understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Program Providers, their employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after use of the Facilities or participation at any Urban Edge LLC/KNPS Program.
